We evaluated our intermediate-term experience with radial urethrotomy and intralesional mitomycin C (MMC) in patients with recurrent bladder neck contractures. METHODS A retrospective study of patients with recurrent bladder neck contractures from Jan 2007 to Jan 2013 was performed. All patients had at least 1 prior failed incision of a bladder neck contracture. Tri or quadrant cold knife incision of bladder neck were performed followed by injection of 0.3 to 0.4 mg/ml MMC at each incision site. RESULTS Thirty-eight patients underwent urethrotomy with MMC injection. All patients had prior endoscopic procedures (range- 1-4). At a median follow up of 12.5 months (range- 11-55 months), 29 (76.3%) patients had a stable, patent bladder neck after one procedure, while 4 (10.5%) patients required 2 procedures to obtain a stable, patent bladder neck. 14 (36.5%) patients presented with an indwelling urethral catheter, and 100% of them had a stable, patent bladder neck after treatment. Eighteen (47.4%) patients presented with incontinence, while twenty-one (55.3%) patients had incontinence after urethrotomy and MMC, with nine patients (23.7%) having placement of an artificial urinary sphincter. CONCLUSIONS Urethrotomy with injection of MMC for the management of recurrent bladder neck contractures is safe and efficacious. Bladder neck patency was demonstrated in 76% of patients after 1 treatment and in 87% after 2 procedures.
