Nous rapportons le cas d’un semi- marathonien qui présente une compression du nerf tibial dans le creux poplité. L’ensemble de la symptomatologie fonctionnelle évoque un syndrome canalaire de type dynamique intéressant le nerf tibial dans le creux poplité de façon bilatérale. Un examen électromyographique fait avant et après effort confirme l’atteinte du nerf tibial avec modification de la conduction nerveuse motrice après effort. La phlébographie révèle une veine poplitée dédoublée avec une compression dynamique du dédoublement latéral de la veine poplitée. L’intervention chirurgicale met en évidence un chef gastrocnémien surnuméraire. Son exérèse permet la rétrocession complète des troubles et la reprise d’une activité sportive à un niveau compétitif. Probablement plus méconnu que rare, le syndrome du nerf tibial piégé dans le creux poplité par un muscle hypertrophié ou surnuméraire s’inscrit dans le cadre plus global du piégeage poplité (« popliteal entrapment »). Les cas de compression isolée du nerf tibial sont très rares. Ceci est probablement dû au fait que les signes cliniques sont peu alarmants et apparaissent dans des circonstances exceptionnelles telles que l’intensification d’une activité sportive. The popliteal artery entrapment syndrome as been covered in several publications. There are however few published cases about nerve entrapment. This report discusses a case of a semi marathonian who shows a tibial nerve compression in the popliteal fossa. The whole functional symptomatology evokes a dynamic, bilateral entrapment of the tibial nerve in the popliteal fossa. To confirm the tibial nerve compression, and its localization before and after effort. Electrodiagnostic studies confirm the tibial compression with a change of motor nerve conduction velocity after plantar flexion of the foot. Phlebography shows a popliteal vein splitting, with a lateral popliteal vein splitting dynamic compression. The surgical intervention shows a supernumerary gastrocnemius. Its ablation enables the complete troubles retrocession, and allows the person to resume high-level competition training. Cases of isolated tibial nerve compression are exceptional. This is probably due to the fact that the clinical signs are a little alarming, and appear in exceptional circumstances such as intense sport activity. Perhaps more often unknown than rare, the tibial nerve entrapment resides in the larger frame of the “popliteal entrapment”." @default.
