- W2160184708 abstract "As part of the preoperative evaluation of patients for liver transplant at our institute, standard echocardiographic examinations are performed to rule out signifi cant cardiac disease. While reviewing these studies, we encountered several patients who exhibited paradoxical expansile motion of the inferior-posterior wall during systole and diastole in the face of overall preserved left ventricle function. The present study analyzes the prevalence, mechanism, and impact of this phenomenon on overall left ventricular (LV) function in patients with liver disease. Patients with end-stage liver disease (ESLD) who had been referred for transthoracic echocardiography as part of the pre-liver transplant workup were identifi ed over an 18-month period. Our center uses the United Network of Organ Sharing criteria for evaluating and listing patients with ESLD. This requires that patients have a Child-Turcotte-Pugh score of 7. All patients underwent echocardiography with commercially available units (Agilent, Andover, Massachusetts; Acuson, Mountain View, California; and ATL, Bothell, Washington) with transducers operating at 2.4 to 3.5 MHz. Patients were studied in the left lateral decubitus position. Two-dimensional, color Doppler, and M-mode images were obtained in the parasternal long- and short-axis and the apical 4- and 2-chamber views. Harmonic tissue imaging was also used at the discretion of the sonographers to obtain images with optimal endocardial defi nition. Studies were recorded on s-VHS videotape for subsequent playback and analysis. Echocardiographic recordings were subsequently reviewed for the presence of any cardiac abnormalities.
