Similar proposals have been put forward in many countries. Although improvements in services from the state and local authorities are feasible, a huge deterrent to success is the public's reluctance to adopt targets and to make the recommended changes in lifestyle.2WHOLeishmania/HIV co-infection. Yet successes are mixed. For example, in the USA, mortality from coronary heart disease (CHD) has almost halved; yet, disappointingly, only a quarter of the fall is attributable to primary prevention.3Gradoni L Scalone A Gramiccia M Troiani M Epidemiological surveillance of leishmaniasis in HIV-1-infected individuals in Italy.AIDS. 1996; 10: 785-791Crossref PubMed Scopus (79) Google Scholar Atherosclerosis and hypertension have not decreased; moreover, obesity is increasing, as it is in all populations. The major falls in smoking in men and women are gratifying, but among adolescents, despite tremendous warnings, the population of smokers is increasing, especially among girls.Turning to other components of lifestyle, consumption of alcohol remains a problem. In respect of an increase in the level of physical activity, urged by all health bodies, in the USA, there is no overall increase as a whole, and in schoolchildren, physical activity is decreasing. Nutrition is another cause for concern. In the USA, although the concept of dietary goals was put forward a generation ago, recent observations indicate that among children, only one in 20 complies with targets in the nutritional pyramid. Among adults, the strongly urged decrease in energy intake from fat, from 40% to 30%, has largely been disregarded. As to the situation in developing countries, in South Africa, in our interethnic populations, the vital statistics of the young have improved substantially.5Marty P Le Fichoux Y Pratlong F Gari Toussaint M Human visceral leishmaniasis in Alpes-Maritimes, France: epidemiological characteristics for the period 1985-1992.T R Soc Trop Med H. 1994; 88: 33-34Summary Full Text PDF PubMed Scopus (68) Google Scholar Yet adult urban Africans (who compose nearly half of the country's African population) already have far surpassed the white population in the prevalence of obesity in women, hypertension, and diabetes. The immigrant Indian population, as elsewhere, has overtaken the white population in mortality from CHD. Tuberculosis is common among the most disadvantaged, and HIV-1 and AIDS in Africans are increasing rapidly. This rising tide of diseases can be stemmed only with the active collaboration of individuals. All endeavours by health authorities to maintain health and prevent disease must, of course, be pressed forward energetically, particularly in relation to health education, in schools, through the media, by posters, &c. But from present attitudes, and the public's general disinterest, the likelihood of extending our years of “health” would seem remote.
