How do we sustain the feeling that what we do is making a difference? How do we maintain job satisfaction in working with the chronically ill? No matter what their occupation, people work to involve themselves in one or more processes or reach one or more goals: earn money, make social contacts, structure time, gain a sense of identity, and give meaning and purpose to activities in life itself. Money is the most concrete reason for working, and successful attainment of that goal is presumably the easiest to measure. However, the ideal income tends to remain elusively at 10 to 20 per cent above current income, even at the very highest socioeconomic strata. Thus depending on income as the primary source of job satisfaction tends to lead consistently to frustration: it’s never enough. If you get pleasure from other aspects of your work, you are far more likely to be content with your income, no matter how high or how low it may be. The importance of work in providing social contacts tends to vary through one’s life span. Even when a confluence of variables, such as accessibility of family and friends, minimizes the importance of work-related social contacts, their effect remains pervasive. Each of us must ask, “ What is the nature of the social contact we seek with the chronically ill? What’s in it for us?” The questions must be answered with the awareness that both the patients and the co-workers with whom we spend our days tend to be different from the kinds of people we might meet in a student health service or an intensive-care unit. As for work’s importance in structuring our time, traditionally our professional activities have provided the major structure to each day, and to the evolving structure of our entire lives. Chronic care tends to lend a more gentle pace to daily activities than acute care, with both persons and events having greater familiarity and providing greater opportunity for growing knowledge and intimacy.
