In pediatric patients with dehydration secondary to gastroenteritis who have failed oral rehydration, does rapid rather than standard intravenous (IV) rehydration result in improved hydration and clinical outcomes? Randomized, double-blind, controlled trial. The Hospital for Sick Children Emergency Department, Toronto, Ontario, Canada. 226 children with hemodynamic stability aged > 90 days with a diagnosis of dehydration secondary to gastroenteritis who failed oral rehydration were enrolled between December 2006 and April 2010. Patients received either a 60 mL/kg (rapid) or 20 mL/kg (standard) bolus of 0.9% saline over 60 minutes, followed by 5% dextrose in 0.9% saline at a maintenance rate for 3 hours. The primary outcome was rehydration at 2 hours, measured by a validated 4 point clinical dehydration scale. Secondary outcomes included the same measure at 4 hours, hospital admission, time in the emergency department (ED), and repeat ED visits within 72 hours. There was no statistically significant difference in the primary outcome between the rapid (36% rehydrated at 2 hours) and standard (29% rehydrated at 2 hours) groups. At 4 hours, 69% were rehydrated in each group. There was a statistically significant difference in the number of patients needing admission (33% rapid vs. 19% standard; P = .04, Number Needed to Treat = 8). The ED length of stay was longer in the rapid group (6.3 hours vs. 5.0; P = .03). This study found no beneficial clinical effects from administration of rapid IV rehydration in children with hemodynamic stability with dehydration secondary to gastroenteritis. Given the potential risks associated with this strategy, it seems prudent to avoid this approach in these patients." @default.
