- W2168454310 abstract "We appreciate the comments by Dr. Spagnolo and his colleagues concerning our article, “Risk of Pneumothorax Not Increased by Obstructive Lung Disease in Percutaneous Needle Biopsy.” When a concept is widely accepted and quoted in many reviews, it usually takes more than one investigation to refute this concept. An increased risk of pneumothorax with obstructive impairment was supported by one clinical study with spirometry, which showed a fourfold increase in 159 patients.1Miller KS Fish GB Stanley JH et al.Prediction of pneumothorax rate in percutaneous needle aspiration of the lung.Chest. 1988; 93: 742-745Crossref PubMed Scopus (88) Google Scholar2Fish GB Stanley JH Miller KS et al.Postbiopsy pneumothorax: estimating the risk by chest radiography and pulmonary function tests.AJR. 1988; 150: 71-74Crossref PubMed Scopus (120) Google Scholar Now two independent studies with pulmonary function data, our study with 93 patients and yours with 89 do not show an increased risk of pneumothorax with obstructive impairment as defined by spirometry. Your study was not referenced in our article as we did not try to rewrite and update the article after its original submission. We also previously published our data in an abstract.3Acevedo-Crespo JC Anderson CLV Fulton D et al.Pneumothorax complicating CT-directed needle biopsy [abstract].Chest. 1992; 102: 177SGoogle Scholar We do not have a good explanation as to why the pneumothorax rate is so variable among different clinical series. We thought that our rate of 35.4% was high but there are reported rates as high as 60%. Patient and lesion selection may be at least as important as technical and operator-related factors. As an example, one clinical series using sonographic guidance had 5 of 124 patients (4.0%)4Ikezoe J Morimoto S Arisawa J et al.Percutaneous biopsy of thoracic lesions: value of sonography for needle guidance.AJR. 1990; 154: 1181-1185Crossref PubMed Scopus (68) Google Scholar with pneumothorax. Such a low rate may be expected in lesions capable of being imaged by ultrasound, which are usually adjacent to solid structures. Obstructive Lung Disease: Percutaneous Needle BiopsyCHESTVol. 107Issue 5PreviewWe read with interest the Anderson et al1 article entitled, “Risk of Pneumothorax Not Increased by Obstructive Lung Disease in Percutaneous Needle Biopsy,” which appeared in the June issue of Chest. We are pleased that the results confirmed our conclusions about this same procedure, which were previously published in Chest.2 Full-Text PDF" @default.
