- W2169838755 abstract "Cerebral small-vessel disease (CSVD) has been shown to contribute to Mild Cognitive Impairment (MCI) and is furthermore considered a risk factor for the conversion from MCI to Alzheimer's Dementia and Vascular Dementia. The mechanisms through which CSVD contributes to cognitive decline are still incompletely understood. A possible mechanism is that CSVD interrupts prefrontal subcortical loops resulting in impaired prefrontal lobe functioning. Functional MRI (fMRI) may detect differences in cortical activity related to the presence of CSVD in MCI. Twenty subjects with established MCI according to the revised criteria of Artero et al. (2006) and fifteen age matched controls underwent fMRI scanning at 3T during a working memory parametric n-back task. MCI patients were subdivided into a group with CSVD (n = 13) and a group without CSVD (n = 7) based on the presence of white matter hyperintensities (Fazekas grade > 2) and/or the presence of one or more lacunes on MRI. FMRI data were analyzed with SPM5 software (Wellcome department, London, UK). After preprocessing at an individual level a second level 1 sample t-test was used to test main effects of working memory load (p <0.001). Second level analysis was furthermore used to calculate group statistics, using a 2 sample t-test adjusted for confounding variables. Main effects showed significant bilateral activation (p <0,001) in the middle, inferior and superior frontal gyrus, the cingulate gyrus, the inferior and posterior parietal lobe (including the precuneus) and the occipital lobe. Controls showed significantly more activation of the working memory network as compared to MCI patient groups. A comparison between the MCI with CSVD and MCI without CSVD groups showed differential activation in the frontal lobe. MCI patients with CSVD showed medial frontal cortex activation whereas MCI patients without CSVD showed middle frontal cortex activation. Our preliminary results, based on 7 MCI patients without CSVD and 13 MCI patients with CSVD, showed differences in prefrontal cortex activation on fMRI during a working memory task. Our results support the notion that MCI is a heterogeneous condition. Presence or absence of CSVD is reflected in differences in prefrontal brain activation in MCI patients." @default.
