Furthermore, the use of antimicrobials prophylactically is generally discouraged except in certain specific circumstances (1-3). Some, but not all, investigators believe that chronic inflammatory disease of the respiratory tract represents one of the situations in which continuous or intermittent prophylactic therapy with broad-spectrum antimicrobials may be beneficial (4-6). In sharp contrast to the reservations concerning prophylaxis in chronic pulmonary disease, there is complete agreement that suspected or proved acute bacterial infection of the lung should be treated promptly and vigorously. The enthusiasm for immediate and appropriate therapy inevitably encourages the administration of amounts of drugs far in excess of those actually needed t.o combat the infection. Justification for the use of large amounts of antimicrobials is based upon the ill-documented thesis that such regimens will produce clinical improvement or will effect eradication of susceptible pathogens more rapidly. One of the risks associated with antimicrobial treatment is superinfection; and one would assume that the incidence of superinfection would be greater if larger amounts or combinations of antimicrobials were used.
