- W2174394843 abstract "【目的】ABTとTCZは代表的なnon TNF阻害薬であるが，これら2つの臨床効果の直接比較は行われていない．日常診療における関節リウマチに対する臨床的効果を比較した．【方法】ABTを投与した194例およびTCZを投与した273例を対象とした．治療選択バイアスを避けるため，propensity score matchingを用いて統計学的に患者を抽出した．【結果】ABT群およびTCZ群の平均年齢60.7歳，59.2歳で，罹病期間は8.7年/9.5年，ベースラインでの疾患活動性はSDAI 28.7/27.7と高い疾患可動性を認めた．52週時点のABTおよびTCZの継続率はABT群71.6％，TCZ群68.6％で差は認めず疾患活動性は，ABT群で18％，TCZ群で20％が寛解に到達し両群に有意差を認めなかった．HAQに関しても，ABT群は36％が，TCZ群では28％が機能的寛解に到達し差は認めなかった．52週のSDAIに寄与する予測因子を検討したところ，ABT群では治療開始前のSDAIが低く，リウマトイド因子が高い例ほど，TCZ群では治療開始前のHAQが低く，bio naiveほど52週におけるSDAIが低値であった．一方HAQについては，いずれの群においても罹病期間が短く，治療開始前のHAQが低い症例ほど52週でのHAQ-DIが低くなることが示唆された．【結語】ABTおよびTCZは日常診療において，同等の臨床効果を持つことが示された．ABTではリウマトイド因子高値例において効果が高く，TCZでは機能障害が出現する前に使用することでより良いアウトカムが得られる可能性がある．" @default.
