- W2174606963 abstract "A 62-yr-old white female was referred to the Hospital of the University of Pennsylvania (HUP) for evaluation of jaundice of 1 month’s duration. She was well until 16 mo before admission, when she suffered a &day episode of painless jaundice, unassociated with fever, chills, dark urine, light stools, itching, nausea, or vomiting. A second similar attack occurred 1 mo later, at which time her total serum bilirubin was found to be 8.1 mg/dl, urine was negative for bilirubin by Ictotest, and alkaline phosphatase and serum transaminases were normal, though lactic dehydrogenase activity was elevated fourfold. After the attack, when her total serum bilirubin had fallen to 3.8 mg/dl (1.6 mg/dl direct-reacting), an upper gastrointestinal series, barium enema, intravenous pyelogram, and oral cholecystogram were obtained. All were reported to be normal. However, ultrasonographic examination of the abdomen revealed a left ovarian cyst and multiple gallstones. Cholecystectomy and left oophorectomy were performed. The pathologic reports were chronic cholecystitis with gallstones (type not specified) and ovarian cystadenoma. She remained asymptomatic until 4 mo before admission, when scleral icterus returned for 1 wk, associated with low-grade fever, coryza, anorexia, and myalgias, but no dark urine or’ abdominal pain.
