- W2182170971 abstract "We evaluated 147patients withadequate color Doppler andangiographic studies for mitral regurgitation. Sixty-five patients hadno mitral regurgitation bybothcolor Doppler andangi- ography and82patients hadmitral regurgitation bybothtechniques. Thusthesensitivity andspecificity ofcolor Doppler forthedetection ofmitral regurgitation was 100%.Three two-dimensional echocar- diographic planes (parastemal longandshort axis, apical four-chamber view) were usedtoanalyze variables ofthemitral regurgitant jetsignals intheleft atrium. Thebest correlation withangiography was obtained whentheregurgitant jet area(RJA)(maximum oraveragefromthree planes) expressed as apercentage oftheleft atrial area(LAA)obtained inthe same plane asthemaximumregurgitant area was considered. ThemaximumRJA/LAAwas under 20%in34of36patients withangiographic grade Imitral regurgitation, between 20%and40%in17of18patients withgrade IImitral regurgitation, and over40%in26of28patients with severemitral regurgitation. MaximumRJA/LAAalso correlated withangiographic regurgitant fractions (r= .78) obtained in21of40patients innormal sinus rhythm andwith no evidence ofassociated aortic regurgitation. Other variables oftheregurgitant jetsuch as maximal linear andtransverse dimensions, maximal area,ormaximal areaexpressed asapercentage of theLAA inone or twoplanes correlated less wellwithangiography. ColorDoppler isa useful noninvasive technique that isnotonlyhighly sensitive andspecific intheidentification ofmitral regurgitation butalso provides accurate estimation ofits severity.
