- W2183675464 abstract "Summary Introduction.Pregnant women with epilepsy are always under high obstetric risk and therefore it is very important to know how to treat them. Purpose.The aim of this study is to present the basic principles of treatment of pregnant woman with epilepsy in general and cases of change in antiepileptic drug concentration in the blood of the pregnant woman and her infant and in the mother's breast milk in particular. Discussion.Recent publications suggest that the percentage of pregnant women with epilepsy in whom increased seizure frequency can be observed is not so high as it used to be thought. Altered pharmacokinetics of antiepileptic drugs is the main reason for increased seizure frequency in preg- nancy. It is very important to monitor blood levels of free fractions of antiepileptic drugs, especially those which bind highly with proteins. Low blood levels of antiepileptic drugs during pregnancy are not always connected with increased seizure frequency. Umbilical blood levels of most antiepileptic drugs (including new generation drugs) are similar to the levels in the mother's blood. No side effects have been observed in the majority of infants, however. The amount of antiepileptic drugs in the mother's food is inversely related to the ability of the drugs to bind with proteins. The total quantity of the antiepileptic drug delivered during breast feeding is usually smaller than the quantity transmitted through the placenta during pregnancy. For this reason, breast feeding is actually rec- ommended for mothers suffered from epilepsy." @default.
