- W2184785770 abstract "SUMMARY Wecompared myocardial infarct size produced bypermanentocclusion ofthemiddleleft anterior descending (LAD)or circumflex (LCx) coronary arteryintheanesthetized dog.Thedogswere killed 3-10dayslater, andtheoccluded coronary bedwas visualized bypostmortemarteriography. The outlines oftheinfarct andoccluded bedweremarked on tracings ofweighed left ventricular (LV)rings and thesize oftheinfarct andoccluded bedwas calculated byplanimetry. Forbotharteries, infarct size and occluded bedsize werelinearly related toeachother, butLAD infarcts were larger relative tooccluded bed size (52.0% vs32.3%, p < 0.05). A smaller occluded bedwas necessaryfortheappearance ofan infarct after LAD occlusion thanafter LCxocclusion (8.3% vs18.5%oftheleft ventricle, p < 0.005). ReconstructedLVring maps indicated asignificantly widermargin ofnoninfarcted myocardium atthelateral edgeof theoccluded bedforLCxinfarcts thanforLAD infarcts. Fordogswithsimilar occluded bedsizes inthe rangeof20-35%oftheleft ventricle, infarct size was considerably larger forLAD occlusion (15.9% vs 6.1%oftheleft ventricle, p < 0.001). Inthis subgroup, blood pressureandheart rate10-20minutes after occlusion were notsignificantly different forthetwoarteries, butcollateral flow, measured with9-,u radioactive microspheres, was approximately 50%lowerafter LAD occlusion. Therelationship between theamountofmyocardium withreduced bloodflowanddeveloped infarct size was similar forthetwo arteries. Weconclude thatocclusions ofthemiddle LAD andLCxarenotequivalent. Foragiven occluded bedsize, LAD occlusions produce larger areasofinfarction, apparently related tolower levels ofcollateral flowdelivered totheoccluded region. ONEofthemostpopular animal models forthestudy ofregional ischemia andinfarction isthedoginwhich oneofthemajor coronary arteries hasbeentemporarily orpermanently occluded. Theanterior descending (LAD)andcircumflex (LCx) branches oftheleft coronaryartery arebothusedforthis purpose andare considered moreorlessinterchangeable. Which branch isusedinagiven laboratory appears todepend moreoncustom thanonanyscientific considerations. Although moreproximal occlusion ofacoronary artery tends toresult inalarger ischemic region anda larger completed infarct, awidevariation isfound in infarct size, evenwith occlusion ataconstant anatomic site. .2 Thisfact stemsfromthevariability incoronary artery anatomy andpertains tobothbranches oftheleft coronary artery.
