Babies requiring special carefrombirth hadahigh incidence ofsporadic diarrhea (36%). Theincidence ofdiarrhea among healthy full-term babies was lowifthey were rooming-in withtheir mothers (2to3%) buthighifthey were housed incommunalnurseries (29%). Themostimportant factor influencing incidence ofdiarrhea was proximity toothernewbornbabies andfrequency ofhandling byunrelated adults. Breast feeding didnotalways protect babies fromdiarrhea. Excretion ofrotaviruses was temporally related to diarrhea in61to76%ofhealthy full-term babies andin44%ofbabies requiring special care.Other enteric pathogens, including enterotoxigenic Escherichia coli, were occasionally isolated. Calculation oftheratios ofsymptomatic toasymptomaticinfection suggests thatbabies requiring special carearemuchmore likely todevelop symptomatic illness after rotavirus infection thanarehealthy full-term babies. During astudy oftheetiology ofdiarrhea in aspecial carenursery ofoneobstetric hospital inMelbourne, Australia, itbecameapparent thatsporadic diarrhea also occurred amongbabiesinotherwardsofthesamehospital (2). Mostofthesebabies werefull-term healthy neonates andwerehoused, handled, andfed differently fromthebabies inthespecial care nursery. Diarrhea seemedtobemorecommon inbabies inthespecial carenursery. We decided tocompare incidence ofdiarrhea inbabies fromdifferent wardsofthishospital during April toJuly(autumn towinter), when theincidence ofdiarrhea inneonates andolder children wasknowntobehigh(3, 6).Etiological agents weresought byculture offeces forenteric bacterial pathogens andbyelectron microscopy todetect noncultivable viruses.
