- W2185454098 abstract "A self-rating scaleofmemoryfunctions was administered to24 non-depressed patients withprobable Alzheimer's disease,divided intotwogroupsaccording totheoverall severity ofdementia(mild, mini-mental state (MMS)>21;moderate, MMS between10and20).Thesegroups didnotsignificantly differ intheirselfratingofmemory functions. The same questionnaire wassubmitted toa memberofeachpatient's family, whohadto ratethepatient's memory.An anosognosiascore wasdefined asthedifference betweenpatient's andfamily's ratings. Thisscorewashighly variable, andcovered,inthetwogroups,thefullrange betweencomplete awarenessofdeficits and totalanosognosia. Correlations betweentheanosognosia scoreandseveral neuropsychological data were searched for.No significant correlation wasfoundwitheither theWechsler memoryscale, theMMS, orlinguistic abilities andgestures. Incontrast, thisscorewas highlycorrelated with the score, defined asthesum ofscoreson theWisconsin cardsorting test(WCST), verbalfluency, Luria's graphicseries, and behaviours (prehension, utilisation, imitation behaviours, inertia, indifference). Amongthesetestsofexecutivefunctions, thehighest correlation withtheanosognosia scorewasobtained ontheWCST.Thissuggests thatanosognosiain Alzheimer's diseaseis not related tothedegree ofcognitive deterioration butresults, atleast inpart,from frontal dysfunction. generally awareofandanxious abouttheir memoryproblems intheearliest stages of theirdisease (forgetfulness phase), they seemtoprogressively loseinsight andtorate their memoryimpairment less severely asthe disease progresses and memoryfunctions deteriorate.5 Fewattempts havebeenmadetoinvestigateawareness ofmemorydeficits inpatients withdementia. Schacter etal6showedthat patients withAlzheimer's disease grossly overpredicted their performance onaverbal learningtaskcomparedwithcontrol subjects. Nearyetal7foundthatdifferent subgroups of patients withAlzheimer's disease showeddifferent levels ofawareness ofdeficits, suggestinga considerable degree ofvariability of anosognosia inthis disease.
