• W2186555148 abstract "Introduction With continuing improvement in surgical techniques and surgical outcomes, the primary focus of elective thyroidectomy has gradually shifted from achieving low morbidity and safety to improving the quality of life or functional outcomes of patients. Unlike the traditional open operation, robotic thyroidectomy (RT) leaves no visible neck scars while it is relatively easier to learn and perform due to flexible robotic instruments relative to non-robotic endoscopic thyroidectomy. It has grown in popularity in some parts of the world particularly those in South Korea and parts of Asia. The experience has been promising, although in October 2011 the FDA revoked the approval on the use of the robot for thyroidectomy. Some of the controversial areas included the surgical and oncological outcomes of RT relative to open and non-robotic endoscopic approaches. Majority of studies had been small, retrospective and single-center, despite that most suggested similar risks to conventional open thyroidectomy and non-robotic endoscopic thyroidectomy. The aim of this critical review was to summarize the current evidence of surgical and oncological outcome of RT, and its application in clinical practice. Conclusion In this review, we found that RT was associated with increased operative time and cost, and may increase risk of temporary recurrent laryngeal nerve palsy and introduce new complications when compared to open thyroidectomy. For patients who prefer the remote-access thyroidectomy, the use of robot may shorten operative time and reduce risk of temporary recurrent laryngeal nerve palsy relative to non-robotic endoscopic thyroidectomy. However, the evidence evaluating the long term oncological outcome remains limited.
