- W2186965980 abstract "Todetermine thehemodynamic andclinical effects oflong-term positive inotropic stimulation on themyocardium, we treated 31patients withseverechronic heart failure withoral amrinone (600mg daily) andperformed invasive hemodynamic studies during short- andlong-term treatment withthedrug. Stroke volumeandstroke workindexes increased markedly during thefirst 48 hroftherapy (p< .01) butreturned topretreatment values after 2to10weeks; upondrugwithdrawal, bothvariables deteriorated rapidly tovalues significantly lowerthanthose observed before treatment withamrinone (p< .01), despite similar values forleft ventricular filling pressure,mean arterial pressure,andsystemic vascular resistance. Thispattern ofresponseindicated thatprogression ofthe underlying heart disease hadoccurred during treatment withamrinone andcontributed importantly to its failure toproduce long-term benefits. Progression ofleft ventricular dysfunction wasassociated with aprogressive increaseinheart rateandplasma renin activity andadecline inserumsodium concentra- tion. Clinically, amrinone therapy was complicated bysustained symptomatic ventricular tachycardia infourpatients, worsening myocardial ischemia infourpatients, andworsening congestive heart failure ineight patients, allofwhomhadbeenstable before entryinto thestudy; onlythree ofthe31 patients improved clinically. Tenpatients diedduring thefirst 2weeksoftreatment, and16(52%) were deadwithin 3months, a mortality ratetwice as great as that seenduring comparable trials with vasodilating drugs. Although noncardiac adverse effects were frequent, they were nottheprimary reasonfordrugfailure. Inconclusion, long-term therapy withamrinone may accelerate progression of left ventricular dysfunction, exacerbate myocardial ischemia, andprovoke life-threatening ventricular tachyarrhythmias, thereby shortening survival inpatients withseverechronic heart failure.
