- W2187887146 abstract "SUMMARY Inorder toestablish whether vestibular abnormalities oftenfoundinspasmodic torticollis aresecondary totheabnormal headposture, thevestibulo-ocular reflex (VOR)was studied ineight patients before andafter correction ofheadposturewithbotulinum toxin. Eyemovements wererecorded inthedarkduring sinusoidal andvelocity steprotation. Fourpatients showeda significantly asymmetric response,withtheslowphaseoftheVOR more active ipsilateral tothe torticollis (chin). Despite significant improvement oftheheadpostureinallpatients forup to10 weeks following treatment, nocorrection ofthevestibular asymmetryoccurred. Thissuggests thatthe VORabnormalities arenotcaused bytheheadpostureitself. Weinterpret thefindings asevidence of primary involvement ofthevestibular systemintorticollis andwe postulate a widespread derangement ofthesensory-motor mechanisms controlling headposture inthis disease. Spasmodic torticollis (ST) isafocal dystonia characterised byinvoluntary movements andpostures ofthe headconsisting ofeither horizontal rotation ortilt in thesagital orcoronal planes, oranycombination of theabove. Noconvincing pathology hasbeen demonstrated,'-3 butitmaybethattorticollis, like other forms ofdystonia, isduetoafunctional abnormality ofneurotransmitter action inthebasal ganglia.4 Thecondition haslongbeenthought tobe associated withabnormalities ofvestibular function.5 Indeed someauthors haveusedtechniques tosuppress vestibular function asamethod oftreatment.6 Central andperipheral lesions ofthevestibular system andits projections areknowntocauseabnormal headpostures, although inhumansthis isexceptional.74 Ina review of35patients with STstudied inourvestibular laboratory, about70%hada directional preponderance ofcaloric/rotational nystagmus contralateral totheside towhich thechinpointed.' Morerecently, measurements ofocular counter-rolling evoked atlow frequency rotation havesuggested abnormalities of otolith function inpatients withspasmodic torticollis.
