- W2188667052 abstract "Thepharmacokinetics ofmetronidazole, itsbiologically active alcohol metabolite, anditsinactive acid metabolite were studied infive noninfected patients undergoing continuous ambulatory peritoneal dialysis andfive patients undergoing hemodialysis. Thelatter were studied on off-dialysis daysas a control group. Peritoneal dialysis caused insignificant changes intheapparent volume ofdistribution, elimination half-life, andtotal bodyclearance ofmetronidazole. Peritoneal dialysis clearance (4.49 ± 0.88ml/kg per h[mean ± standard deviation]) accounted foronly8.9%oftotal bodyclearance (50.17 ± 18.64 ml/kg perh).Analysis ofthe24-h areaundertheserumconcentration versustime curvesandperitoneal dialysis clearance datafor thetwometabolites suggested a similar insignificant effect ofperitoneal dialysis on their elimination. Metronidazole dialysate concentrations inthefirst 6-hexchange ranged from7.6to11.7,ug/ml. Thiswould suggest thatcumulative penetration ofmetronidazole fromthesystemic circulation intotheperitoneal cavity withdosing every8 h should leadtoadequate concentrations forthetreatment ofanaerobic peritonitis. Forthetreatment ofsystemic anaerobic infections, itwould appearatpresentthatmetronidazoledosageadjustments are notnecessaryinpatients undergoing continuous ambulatory peritoneal dialysis. Thepotential formetabolite accumulation was noted inthis study. Iffurther studies confirm that excessive serum metabolite concentrations aretoxic, dosage reduction inthis group ofpatients may be warranted. Metronidazole (MT)isanitroimidazole compound which hasbeenwidely usedinthetreatment oftrichomoniasis, amebiasis, andgiardiasis (7). Manystudies havealso documented theefficacy ofMT inthetreatment andprophylaxis ofanaerobic infections (2). Twenty tothirty percent ofan administered doseisexcreted intheurine astheparent compound. Theremainder ismetabolized intheliver via conjugation andoxidation, withsubsequent elimination of themetabolites intheurine andfeces (13).
