Se trata de un estudio descriptivo de corte transversal, llevado a cabo en el Hospital Simón Bolívar de Bogotá, entre octubre de 2007 y octubre de 2008. Se incluyeron pacientes menores de 36 meses con fiebre en quienes, después de la historia clínica y el examen físico completo, no fue posible establecer el origen de la fiebre. A partir de las historias clínicas se obtuvo información respecto a las características demográficas y clínicas, la frecuencia y los resultados de las pruebas de laboratorio, y el desenlace. Según el resultado, los pacientes se clasificaron en tres grupos: enfermedad bacteriana grave, infección focal benigna y posible infección viral. Se analizaron 101 pacientes, correspondientes al 1% de todas las consultas pediátricas del año. El promedio de edad fue de 11,6 meses, y el 86% de los pacientes estuvo entre los 3 y los 36 meses. Se obtuvo hemograma en el 91% de los pacientes, parcial de orina en 86,1 %, Gram de orina en 62,4%, y cultivos de sangre y de orina en 7,9% y 13,9%, respectivamente; en 42 pacientes se ordenó radiografía de tórax. La causa de la fiebre sin foco aparente se distribuyó así: 68 %, posible etiología viral; 25%, enfermedad bacteriana grave, y 7%, enfermedad focal benigna. En el grupo de enfermedad bacteriana grave, se encontraron infección de vías urinarias (11,9%), neumonía de posible etiología bacteriana (10,9%) y meningitis (1%). Este estudio es el primer paso para la aproximación real a este tipo de paciente en condiciones locales y plantea múltiples retos para futuras investigaciones. Fever without an apparent focus in children under 36 months is a frequent pathology. So far there is a knowledge gap about the characteristics of this entity in our setting. To determine the prevalence of fever without an apparent focus in children less than 36 months of age, its causes and characteristics, and the practices toward this condition. A cross-sectional descriptive study at the Hospital Simón Bolívar in Bogotá between October 2007 and October 2008 was carried out, with patients under 36 months of age, with fever without an apparent source; it was not possible to establish the origin of fever after medical history and complete physical examination. Information regarding clinical and demographic characteristics, frequency and results of diagnostic and outcomes was obtained from medical records. Patients were classified according to the outcome into three groups: severe bacterial disease, benign focal infection, and possible viral infection. On hundred and one patients (representing 1% of all pediatric visits per year) were analyzed. Mean age was 11.6 months, and 86% of patients were between 3 and 36 months old. Blood counts were obtained in 91% of patients, urine analysis in 86.1%, Gram of urine in 62.4%, and blood or urine cultures in 7.9% and 13.9%, respectively. Forty two patients had chest X-rays. The cause of fever without focus was distributed as follows: 68%, possible viral etiology; 25%, severe bacterial disease, and 7%, benign focal disease. Severe bacterial disease comprised urinary tract infection (11.9%), pneumonia of likely bacterial etiology (10.9%), and meningitis (1%). This study is a first step for approaching this condition at local settings, and poses many challenges for future researchers." @default.
