• W2210103936 abstract "Incretin hormones play a key role in the regulation of postprandial insulin release. Glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) account for approximately 90% of the incretin effect, but only GLP-1 has been shown to play a role in the control of fasting glycemia. In addition to its insulinotropic actions, GLP-1 has other glucose-lowering effects including inhibition of glucagon secretion, hepatic glucose production, gastric emptying, and appetite. Studies in mice and pancreatic islets lacking ATP-sensitive potassium (KATP) channels point to a role of GLP-1 and its receptor in the pathophysiology of KATP hyperinsulinism (KATPHI). Particularly, the GLP-1 receptor antagonist exendin-(9-39) inhibits insulin secretion and corrects fasting hypoglycemia in SUR1-/- mice, suggesting a potential therapeutic application of this peptide in the treatment of KATPHI. In addition, early evidence in individuals with postprandial hyperinsulinemic hypoglycemia after gastric surgery suggests that GLP-1 may also play a causal role in this entity. The GLP-1 receptor has been used successfully in the localization of pancreatic insulinomas with radiolabeled peptides. Future studies are needed to examine the potential application of the GLP-1 receptor as a therapeutic target for congenital and acquired forms of hyperinsulinemic hypoglycemia." @default.
