- W2211041959 abstract "Patient with HIV infection have increased risk of cardiac involvement. Pericardial involvement is one of the cardiovascular manifestation of HIV infection. Most of the studies which have been done before were done in the pre HAART therapy era. The purpose of the current study is to find out the prevalence and the impact of pericardial involvement in the post HAART drug therapy era among African Americans with HIV infection. The introduction of HAART drug therapy has brought a change in the prevalence of pericardial involvement among HIV patients. A retrospective analysis of 11845 patients with HIV infection who were seen in the Bronx Lebanon hospital and it's affiliated clinics from 2006 to 2014 was done to evaluate the presence of pericardial involvement. Out of these 5893 patients were found to be African Americans. (males-3321; females-2572). Mean age of all the patients who were analysed was around 50. Both inpatients and outpatients were analysed involving patients of all age and both sexes. Pericardial effusion diagnosis was made by using Echocardiogram supported by EKG findings. Out of the 5893 HIV infected African American patients analysed 33 (0.55%) patients were found to be having pericardial involvement. Out of these, 20 (0.60%) were males and 13 (0.50%) were females. There is no gender predisposition for pericardial involvement in HIV infected African American patients. (p value>0.05).Out of the 33 persons with pericardial involvement, 27(82%) were found to be having AIDS. The mean CD4 count among the HIV infected individuals with pericardial involvement was 104.Out of these 33 persons,4 were found to be having cardiac tamponade (0.06%). Most of the pericardial effusions documented were small effusion (70%). Moderate effusion was seen in 12%; Large effusion was seen in 9%.The average CD4 count of the entire sample was 104. (p value <0.05) implying that lower CD4 count level does correlate with the occurrence of pericardial involvement in HIV infected African American patients. HIV related pericardial involvement in the current post HAART era, as revealed by this study is lower than the data obtained from other pre HAART era studies. This is the first study done exclusively among African Americans. This implies that the HAART therapy and the subsequent improved levels of CD4 count in these patients has brought down the incidence of pericardial involvement in HIV patients. The prevalence of pericardial involvement is similar to the study done in Germany in the current HAART era which reported a prevalence of 0.25%.Also the mean CD4 count of the patients who developed pericardial involvement was lower than the mean CD4 of the patients who did not have pericardial involvement. (p value_0.009). This implies that higher the immunosuppression more is the tendency for pericardial involvement in HIV infected patients.
