Thus, I have two suggestions – the first is that not only should applicants be honest with themselves and everyone else about their career track, but fellowship programs should also be true to themselves. Research oriented programs should focus on research oriented fellows (clinical, bench, translational or a combination, depending on the expertise of the program and institution) and clinically oriented programs should focus on excellent clinicians. Of course, some overlap is healthy, but everyone should be clear about their primary focus and goals.For the prospective fellow truly interested in research, carefully evaluate the program - is the program director accomplished in research or does s/he have a strong commitment to support research? Is the faculty enthusiastic about research, and are they available? What is the faculty track record of publication with fellows? What jobs do the fellows have after completion of their fellowship? Does the program support additional degrees such as an MS degree (eg, in outcomes research, biomedical engineering, statistics) or an MPH? And, how will the fellowship program specifically support your career in research? Getting these questions answered early should help everyone.
