• W2222005304 abstract "In den Jahren 1982 bis 1993 wurden insgesamt 92 Patienten (58 Manner, 34 Frauen) in den Stadtischen Kliniken Duisburg wegen einer Arteriovenosen Malformation des Gehirns einer Strahlenbehandlung in fraktionierter Technik unterzogen. Hierbei handelte es sich ausnahmslos um inoperable Falle oder um Patienten, die eine Operation ablehnten. Dementsprechend waren hohergradige Angiome uberreprasentiert (Spetzler-Grade: I°: 3,3%, II°: 19,6%, III°: 32,6%, IV°: 22,8%, V°:21,7%). Bei 32,6% der Falle betrug der maximale Durchmesser bis drei Zentimeter, bei 31,5% zwischen drei und sechs Zentimeter und bei 35,9% uber sechs Zentimeter.Die Strahlenbehandlung wurde vorgenommen an einem Linearbeschleuniger der Grenzenergie 10MeV in Einzelfraktionen von 2Gy funfmal wochentlich bis zu einer Gesamtdosis von 50Gy (65 Patienten). Eine kleine Anzahl von Patienten erhielt auch ein alternatives Konzept: Gesamtdosis 20Gy in 4x5Gy in einer Woche (23 Patienten), Gesamtdosis 48Gy in Einzelfraktionen von 4Gy 2x wochentlich (2 Patienten), Gesamtdosis 48Gy bei Einzelfraktionen von 4x2Gy pro Woche (1 Patient) und Gesamtdosis 51Gy in 3x3Gy pro Woche (1 Patient).Kontrolluntersuchungen wurden angiographisch, computertomographisch und kernspintomographisch vorgenommen. Insgesamt blieben 70% der Angiome unverandert. Nur in 11,3% der Falle konnte eine vollstandige Obliteration der AVM nachgewiesen werden. In den restlichen 18,7% stellte sich die Gefasmisbildung verkleinert dar. Hierbei entsprachen die Resultate des 50Gy-Konzepts denen des 20Gy-Konzepts. Totalobliterationen wurden fast ausschlieslich in der Untergruppe der Angiome beobachtet, deren Durchmesser unter drei Zentimetern betrug. In dieser Gruppe der kleinen AVM wurde bei 29,2% der Patienten ein vollstandiger Verschlus der Lasion festgestellt.Nur Patienten mit vollstandig obliterierten Lasionen waren vor weiteren Angiomblutungen gefeit. In der Untergruppe der kleinen AVM (<3cm) zeigte sich eine Tendenz zur Abnahme des Blutungsrisikos, die jedoch statistisch nicht signifikant war. Demgegenuber stieg das Blutungsrisiko bei den Angiomen mit einer Grose von mehr als drei Zentimetern nach der Radiatio noch statistisch signifikant an. Auch in den Fallen, in denen eine vollstandige Obliteration der Lasion ausgeblieben war, zeigte sich ein signifikanter Anstieg des Risikos einer Angiomblutung.Heutzutage ist die konventionell fraktionierte Strahlenbehandlung zerebraler Arteriovenoser Malformationen aufgrund ihrer schlechten Ergebnisse zugunsten der stereotaktischen Einzeittherapie verlassen worden. Auch konnten auf dem Gebiet der neurochirurgischen Mikrochirurgie und der Embolisationstechnik entscheidende Fortschritte errungen werden. Die vorliegende Studie hat insofern eher historischen Wert. Sie ist jedoch die groste und einheitlichste Studie uber konventionell fraktioniert bestrahlte Angiome in der verfugbaren Literatur." @default.
