- W2230052007 abstract "Objective: To investigate the novice user's experience modelling new concepts in the context of mapping an existing medical terminology to SNOMED CT. Thus gaining an understanding of the difficulties the novice has with SNOMED CT which can inform the focus of future tooling development. Background: Large-scale adoption of SNOMED CT promises to help with the problem of data standardisation and therefore aid data interoperability, but mapping an existing medical terminology to SNOMED CT is not simple. SNOMED CT has a series of Style Guide documents that define how a correct SNOMED CT expression may be written. Snapper is a tool that tries to ease the burden of modelling new concepts when mapping, but it has room for improvement. This investigation aims to reveal problem areas that tools like Snapper could address. Methods: A data analyst experienced with health related datasets, but not SNOMED, was asked to map the ANZICS (intensive care) diagnosis codes to SNOMED CT. They had a session with an ANZICS expert and were given basic SNOMED compositional grammar training, as well as basic training for Snapper, before being left to complete the mappings individually. An interview was conducted to review the mappings and discuss the issues that were encountered. Results: A good understanding of the SNOMED CT Style Guide is crucial to producing good mappings - and the quality of the data analyst's mappings improved as their knowledge of the Style Guide did. A terminology like ANZICS is particularly challenging to map with no experience, as the concepts are wide ranging and require an extensive knowledge of the Style Guide. The data analyst's efforts were hampered by a lack of knowledge of SNOMED CT and the SNOMED Composition Grammar. All of the expressions that were checked for correctness using Snapper's constraint checker were found to be incorrect, however, the display of the checker helped the data analyst to understand where the problem was, and more importantly, how the problem could be fixed. Conclusions: It is vital to be able to check expressions for correctness as this provides the confidence to continue and to attempt more difficult mappings. Immediate feedback also allows for correct mappings to be created from the start and reduces the need to review and re-map existing mappings. Not all aspects of the Style Guide can be machine checked as editorial guidelines are not machine-readable. To handle semantic constraints, support for a peer review approach to checking expressions is a desirable feature for tools. Other desirable features include a mechanism for supporting the hierarchy that exists in terminologies to maintain the context, and further support around the SNOMED Compositional Grammar to highlight syntax problems. Further work on the result display of Snapper's constraint checker would improve its usability, especially for more complex expressions." @default.
