InBrazil, KPCwas initiallydescribedinK. pneumoniaein Recife,state of Pernambuco,in2006, butcurrently this enzymeisalreadydisseminated throughout the country, where itsincidence has increased significantly.Thus, this study aimed to perform the molecularcharacterization of KPC-producing brazilian isolates belonging to different species ofEnterobacteriaceae (non-K. pneumoniae) originated from different Brazilian states between2009 and 2011. The resistance profile was evaluated by disc-diffusion method and E-test.The allelic variant of theblaKPCgene, as well as the participation of Tn4401and the presenceof other beta-lactamase genes (TEM, SHV and CTX-M) were analyzed by PCR and genomesequencing. Plasmid analysis and hibridization were used to determine the geneticenvironment of theblaKPCgene. Molecular typing was done by PFGE and MLST (only forEscherichia coli).Eighty threeunique clinical isolates of EnterobacteriaceaeKPC-2-producerswerereferred to the Laboratorio de Pesquisa em Infeccao Hospitalar fromFundacao Oswaldo Cruz,correspondingto 9 different species (Enterobacter aerogenes,Enterobacter cloacae,Escherichia coli,Pantoea agglomerans,Providencia stuartii,Citrobacter freundii,Klebsiella oxytoca,Morganella morganiiandSerratia marcescens)isolated from 9 states located in northeast, southeast and central regionsof Brazil. Highresistance rates towards most of the antimicrobial agents tested, including tigecycline (36.1%nonsusceptible) and polymyxin B (16.5%) were detected.E. aerogenes,E. cloacae,C.freundiiandK. oxytocawere the most resistant species. The association of theblaKPC-2genewith other beta-lactamase genes was found in 53% of the isolates (45.8% producing TEM,6% producing SHV and 22.9% producing CTX-M). TheblaKPC-2gene was found associatedwith a partial Tn4401structure with different isoforms. All of the isolates had theblaKPC-2gene inserted within plasmids of different sizes and incompatibility groups. We found greatclonal diversity among all of the studied species, but we found the presence of a fewprevalent clonal groups in some regions. The majority ofE. aerogenesisolates belonged toclonal group A, isolated from the central regions of Brazil (GO and DF), persisting for 11months, suggesting the possibility of an outbreak. Therefore, this study demonstrated thedissemination of theblaKPC-2gene among different Enterobacteriaceae in Brazil,includingspecies in which this gene wasnot previously reported, such asP. agglomeransandP. stuartii.The study alsoshowed the different molecular tools in which theblaKPC-2genebenefits to disseminate betweenEnterobacteriaceae." @default.
