- W2233181562 abstract "The e$ect of combined dopamine (0) and nitroprusside (NP) drug therapy with preload restoration was assessed in 15 patients with depressed lef2 ventricular (L V) function ear& following various intracardiac operative procedures. Low-dose D (2.6 f 0.7 ug/kglmin) significantly (P < 0.01) increased cardiac index (Cl) by 27% and stroke work index (LVSWI) @ 30% and reduced vascular resistance (SVRI) by 23% without any signijcant change in mean arterial pressure (Kp) or preload (LAP). .The addition of low-dose NP (0.5 + 0.2 peglmin) to D significant@ (P < 0.01) further reduced SVRI b 15%, LAP by 25%, and AP by 11% in association with a nominal increase in CI and decrease in L VSWI. Such combined drug therapy produced a synergistic, salutary effect on L V performance; moreover, this e$cct represented functional reclamation of “preload reserve. ‘T Acquisition of the potential hemodynamic benefits (in terms offorward cardiac output) inherent in this reclaimed “preload reserve” simply required restoration of preload to control level. The net result of combined D + NP therapy plus preload restoration (compared to control) &as a 45% innease in CI (P < O.OI), a 30% increase in LVSWI (P. O.Ol), a 41% decrease in SVRI (P < O.OOI), and 10% decrement in AP(P < 0.01). In comparison to the values obtained with D alone, addition of NP and restoration of preload resulted in a II% increment in CI (p= O.OOI) and a 24% decrease in SVRI (P < 0.001) without any sign$icant change in LAP or LVSWI. Double product did not change signifwantly during any intervention state. Combined D + NP therapy phs preload restoration favorably shi$s L V performance to near maximal levels at relativeb in?onsequential cost in terms of myocardial oxygen demand in postoperative patients. Additionally, combined D + NP therapy minimizes the limitations to individual drug treatment and results in optimal hemodynamic manipulation." @default.
