- W2236007265 abstract "Thyrotoxic periodic paralysis (TPP), a hyperthyroidism related hypokalemia and muscle-weakening condition resulting from a sudden shift of potassium into cells, many patients with TPP have no obvious symptoms related to hyperthyroidism. Therefore, several important clues may help in diagnosing and managing TPP: presentation in an adult male with no family history of periodic paralysis; presence of systolic hypertension, tachycardia, high QRS voltage, first-degree atrioventricular block on electrocardiography; presence of low amplitude electrical compound muscle action potential on electromyography and no notable changes in amplitudes after low doses of epinephrine; electrolyte findings such: hypokalemia with low urinary potassium excretion, hypophosphatemia associated with hypophosphaturia, and hypercalciuria. Immediate therapy with potassium chloride supplementation may foster a rapid recovery of muscle strength, but with a risk of rebound hyperkalemia. Nonselective β-blockers may provide an alternative choice. Long-term therapy with definite control of hyperthyroidism completely abolishes attacks. We present our experience with four cases from Hospital Angeles Leon. Resumen La paralisis periodica tirotoxica es una condicion caracterizada por debilidad muscular generalizada relacionada con hipocaliemia e hipertiroidismo, ocasionada por un repentino secuestro de potasio intracelular; muchos de los pacientes no presentan sintomas de hipertiroidismo, sin embargo los datos que sugieren el diagnostico son: presentacion en hombre adulto, sin antecedentes de historia de paralisis periodica familiar y que pueden cursar con hipertension sistolica, taquicardia, complejos QRS altos o bloqueo auriculoventricular de 1er. grado al EKG, asi como la presencia de potenciales electricos de baja amplitud durante la contraccion muscular en la electromiografia, sin cambios con dosis bajas de epinefrina, aunado esto a hipocaliemia, hipofosfatemia, hipocaliuria, hipofosfaturia e hipercalciuria; la terapia con cloruro de potasio revierte rapidamente a la paralisis, sin embargo existe el riesgo de hipercaliemia, por lo que el uso de β bloqueadores puede ser el tratamiento de eleccion, ademas el control del hipertiroidismo soluciona definitivamente el cuadro mencionado.
