- W2247233238 abstract "3679 Background: Endorectal ultrasound is frequently used to select patients with rectal cancer for neoadjuvant therapy. The ultrasound results can be difficult to interpret after chemoradiation. The purpose of this study is to determine the accuracy of transrectal ultrasound before pre-operative chemoradiation in predicting survival and after pre-operative chemoradiation in predicting the pathological stage of patients with rectal cancer. Methods: Patients with clinical stage II or III rectal cancer 0 to 15 cm from the anal verge had transrectal ultrasound before and after receiving a standardized regime of external beam radiation, 5-Fluorouracil and CPT 11. Patients then underwent surgical resection, followed by additional treatments with 5-Fluorouracil and Leucovorin. Ultrasound results before chemoradiation were compared to survival. Ultrasound results after chemoradiation were compared to surgical pathology. Results: 37 patients who were enrolled in the study with clinical stage II or III rectal cancer underwent both pre and post chemoradiation transrectal ultrasound examinations. On pre-chemoradiation ultrasound, 2 patients were found to be stage I, 24 patients were stage II, 11 patients were stage III. Patients were followed from 1 to 6 years. At 3 years follow-up, both stage I patients were alive, 92% (22/24) of patients with stage II cancers were alive (1 died, 1 lost to follow-up), 73% (8/11) of stage III patients were alive (1 died, 2 lost to follow-up). Post chemoradiation ultrasound correctly predicted pathology in 24% (9/37) patients. Tumor status was correctly predicted in 41% (15/27), with 51% (19/37) being overstaged. Nodal status was correctly predicted in 70% (26/37) of patients, with 24% (9/37) of patients understaged. Conclusions: Transrectal ultrasound is a valuable tool to select patients for neoadjuvant therapy. After chemoradiation therapy, ultrasound is of limited value in predicting pathological stage.
