- W2247256626 abstract "16065 Background: Metastatic choriocarcinoma is the most aggressive subtype of nonseminomatous germ cell tumors (NSGCT) that tends to early spread into viscera, especially lungs. Resection of all post-chemotherapy residual tumors has become a standard option in metastatic NSGCT. However multiple metastases of choriocarcinoma frequently involve retroperitoneal lymphnodes, liver, and both lungs. In this setting it is difficult (and often just impossible) to achieve “no evidence of disease” (NED) status. We performed an analysis to review the outcome of patients (pts) with choriocarcinoma who became marker-negative after induction chemotherapy (CT) with their residual PM left. Patients and Methods: 43 CT-naïve pts with metastatic NSGCT, PM and baseline hCG level > 50000 IU/l were treated in our center from 1993 to 2006. 20 out of them completed induction CT, became marker-negative and still had residual PM. Median pre-treatment hCG level was 250000 IU/l (range, 54900–3260000). 18 (90%) pts had testicular primary tumor. Induction CT-regimens were T-BEP (40%), BEP (40%), C- BOP-3BEP (20%). Median follow-up was 42 (range, 14–107) months. Results: 15 (75%) out of 20 pts had residual tumor resections. 4 out of them underwent resections of two metastatic sites (RPLND - 10 pts, lung resections - 6 pts, liver resections - 3 pts). No viable tumor was found in resected masses. Mature teratoma arose more often in RPLN - 5 (50%) pts compared with lung metastases - 1 (17%) pt (it was a solitary residual PM). Despite surgery 17 (85%) pts could not achieve NED status mostly due to nonresected multiple residual PM. Two (10%) pts relapsed: occurrence of brain metastases in both of them without progression of PM (one pt previously underwent resection of a gross PM and second one - RPLND). Conclusion: Resection of PM has no influence on patients’ outcome in metastatic choriocarcinoma pts with multiple residual PM, who achieved marker-negative status after induction CT.
