Cette revue de la littérature met en évidence la complexité de la notion d’insight dans le cadre de la schizophrénie. L’absence de consensus dans la définition de l’insight amène à penser l’insight dans la schizophrénie comme un phénomène complexe. L’une des conséquences est alors la difficulté à l’évaluer de façon standardisée. Si la conscience de la maladie apparaît comme un enjeu dans la prise en charge des patients, aucun consensus ne se dégage dans les effets positifs ou délétères qu’un bon degré d’insight pourrait avoir sur le patient, son trouble et sa prise en charge. Afin de mieux comprendre les effets de l’insight, dans un travail en cours, des patients atteints de schizophrénie à des degrés divers de rémission seront interrogés avec des outils multidimensionnels et un entretien standardisé. L’objectif sera de confronter non seulement les points de vue (experts/patients) sur ce qu’est l’insight mais aussi de déterminer de quelles façons les dimensions spécifiques de l’insight pourraient être importantes dans la prise en charge des patients atteints de schizophrénie. This work consists in a literature review highlighting the different conceptions and the different modes of evaluation of insight among patients suffering from schizophrenia. The purpose was to show that insight is a phenomenon with numerous facets and dimensions. The different conceptions and evaluation methodologies will be studied individually so as to show how complex the insight phenomenon is and how difficult it is to evaluate it in a multidimensional manner, and to determine how this relates to the specific dimensions involved in the evolution and the care of schizophrenic disorder. This review of the literature highlights the complexity of insight in schizophrenia. The absence of consensus on the definition of insight suggests that, in schizophrenia, it is a complex phenomenon. One of the consequences is then the difficulty in assessing it in standardized manner. While awareness of the illness appears to be an important issue in patient care, no consensus exists on the possible positive or deleterious effects a good level of insight on the patient, his/her disorder and his/her care. In order to understand the effects of insight on on-going work, in future research patients suffering from schizophrenia at different stages of remission could be submitted to multidimensional measures and standardized interviews.
