Im Rahmen der visuomotorischen Imaginationstherapie wurden Positionen und sequentielle Ablaufe des kontralasionalen Armes wiederholt so lebendig und intensiv wie moglich mental geubt. Detaillierte Instruktionen und ein adaptives Vorgehen sollten eine intensive kinasthetische Imagination gewahrleisten. Die Machbarkeit wurde anhand von Therapieprotokollen und elektromyographischen Ableitungen erfasst. Die Effektivitat der Imaginationstherapie wurde anhand einer ausfuhrlichen neuropsychologischen Testbatterie uberpruft. Funfzehn subakute Neglektpatienten der Kliniken Schmieder Heidelberg erhielten eine dreiwochige Imaginationstherapie als Add-On-Verfahren und wurden mit funfzehn Neglektpatienten verglichen, die keine zusatzliche Therapie zur neurologischen Rehabilitationsroutine erhielten. Zehn Patienten mit chronifiziertem Neglekt nahmen an einer vierwochigen Einzeltherapie im Rahmen eines Baseline-Designs mit katamnestischer Nachuntersuchung teil. Zum eigenstandigen Weitertrainieren wurde den Patienten eine Audio-CD mit Instruktionen fur visuomotorische Imaginationsubungen ausgehandigt. Bei neun chronischen Neglektpatienten wurde vor der Interventionsphase eine funktionell-bildgebende Untersuchung durchgefuhrt zur Erfassung der generellen Imaginationsfahigkeit bei Neglekt. Die Therapieprotokolle wiesen auf eine hohe Compliance und subjektive Gewinne in der Wahrnehmung der linken Raum- und Korperhalfte bei allen Patienten hin. Kinasthetische Imagination war von sensorischen Empfindungen und muskularer Aktiviat begleitet. Bei der Imagination von Bewegungen der betroffenen Hand wurden verstarkt kortikale Areale der kontralasionalen Hemisphare rekrutiert. Proportional zur Eingangsleistung verbesserten sich die subakuten Neglektpatienten im Vergleich zur Kontrollgruppe deutlich mehr beim Abzeichnen, in der Korperwahrnehmung und der Armsensorik sowie –motorik. Signifikante anhaltende therapieinduzierte Verbesserungen zeigten sich in der Exploration, der kinasthetischen Imagination und Alltagsleistungen. Das Eigentraining wurde als weniger intensiv empfunden als die Therapie mit personlicher Assistenz. Visuomotorische Imagination ist durchfuhrbar bei Neglektpatienten im subakuten und chronischem Krankheitsstadium. Schmerzen und depressive Verstimmungen stellten Kontraindikatoren dar. Perspektivisch sollen psychometrische Instrumente zur Beurteilung der individuellen Imaginationsfahigkeit, Ein- und Ausschluskriterien fur die Patientenauswahl, Methoden zur statistischen Effektivitatsuberprufung der visuomotorischen Imagination, sowie Eigentrainingstechniken ausgearbeitet werden." @default.
