- W2261939655 abstract "Intercurrent fungal infection constitutes a small but definite hazard during long-term steroid therapy. While Candida, Mucor, and Aspergillus have most often been implicated (1), an ever-increasing body of evidence is accumulating which suggests that administration of corticosteroids may be capable of enhancing occult pulmonary cryptococcosis (2-4). Although the clinical evidence appears to be convincing, experimental studies, which have shown variable results, have carefully avoided the natural (respiratory) challenge route (5-10). In addition, some workers (8-10) have chosen the intraperitoneal route of corticoid administration, which was shown by Greenspan, Gifford, and Deming (11) to reduce substantially the amount of active steroid reaching the tissues, thus further complicating the interpretation of these studies. It is generally agreed that steroids interfere with the mechanisms that localize infection (including decreased cellular exudation, decreased phagocytic mobilization and intracellular digestion, suppression of capillary dilatation) (12) and, at the same time, depress the synthesis of specific gamma globulin (13, 14) in some lower animals. As these factors are considered of major significance to host resistance against Cryptococcus neoformans (8, 15-19), it would be expected that high doses of cortisone might allow consistently earlier dissemination of organisms from a pulmonary locus. It is the purpose of this report to assess the role of cortisone in experimental cryptococcosis in the rat and guinea pig as reputed representatives of cortisone-susceptible and cortisone-resistant species (20)." @default.
