- W2263138307 abstract "Type 1 diabetes had a major scientific breakthrough in 1921, when Frederick Banting and Charles H Best discovered the hormone insulin in pancreatic extracts of dogs. In 1922, the first injection of insulin given to a human being saw the prognosis for type 1 diabetes change from a death sentence to a condition that could be managed. So why, nearly a century later, is access to insulin still a global health problem? David Beran (University of Geneva and Geneva University Hospitals, Geneva, Switzerland) is driven in his work by the global inequity in diabetes care—depending on your country's health system, your chances of survival can range between 1 year and near-normal life expectancy. Constraints and challenges in access to insulin: a global perspectiveSubstantial attention has been given to the issue of access to medicines for communicable diseases; however, access to essential medicines for diabetes, especially insulin, has had insufficient focus. Although insulin was discovered in 1921, the drug is unattainable to many globally, and this Review aims to highlight the range and complexity of factors that contribute to this unattainability. Manufacturers' selling prices of various insulin formulations and presentations, duties, taxes, mark-ups, and other supply chain costs affect the price of insulin and hence the drug's affordability to health systems and individuals.
