• W2267208547 abstract "Congenital glaucoma is one of the main causes of blindness in children. Its pathogenesis is still disputed. Up to now, medical therapy for congenital glaucoma is accorded a supportive role; the primary, definite treatment is surgery. The initial procedures of choice are goniotomy and trabeculotomy, and both give good results in the studies in western countries. During the year 1984-1998, 11 patients (15 eyes) diagnosed as having congenital glaucoma at our hospital were enrolled in this study. This paper presents our surgical experience of those patients who had a follow-up of at least 6 months. In our series, goniotomies were performed before 1992, and trabeculotomies were performed after then. For surgery to be considered a success, an eye had to demonstrate (1) an IOP of 20 mmHg or less without medication, (2) stabilization or improvement in the optic disc appearance, and (3) absence of further corneal enlargement disproportionate to normal growth. After a single goniotomy procedure, 2 (28.6%) of the 7 eyes (5 patients) in this group fulfilled the criteria of successful outcome. On the other hand, of the 7 eyes (6 patients) who underwent trabeculotomy, 6 eyes (85.7%) had a successful outcome (One eye was excluded due to transferring to trabeculectomy during the operation). The difference in success rates between the two groups was statistically significant (P＜0.01). Our results, and other series, suggest that successful management of congenital glaucoma may be achieved utilizing fewer procedures when trabeculotomy is employed rather than goniotomy." @default.
