- W2274688051 abstract "Oral rehydration using glucose/electrolyte and sucrose/electrolyte solutions has been effective in young children with diarrheal dehydration. Premeasured packets of sugar/electrolytes have not been made available at the rural village level in less developed countries. Simple solutions prepared by mothers using household salt and sugar have been advocated. However under supervised conditions many mothers have prepared solutions that were hypertonic in sodium. A method was devised and tested that used a 5 ml teaspoon and a 1 liter container as standard units of measure commonly available at the village level. The method was first tested under laboratory conditions. 1 level teaspoon of salt and 4 heaping teaspoons of sugar in 1 l of water provided a solution containing 72-78 mmol/1 liter of sodium and 3.2 gm % of sucrose. After 10 trials the weights of the sugar and salt varied minimally. 5 US nurses prepared quadruplicate solutions in 1 liter of water using 1 teaspoon of salt and 4 heaping teaspoons of sugar. The mean sodium concentration was 74 +or- 8 mmol/l and the mean sucrose concentration was 3.3 +or- .3 gm %; both showed minimal variations. The method was field tested in Honduras with 20 mothers each instructed to bring a teaspoon and a 1 liter container from their homes. Under supervision they were instructed in the method for preparing the solution. 16 of the mothers brought the appropriate sized implements. Sodium levels in the solutions ranged from 60-145 mmol/l; 16 solutions were in the 60-95 mmol/l range. Sucrose concentrations were more variable ranging from 1.3 to 3.7 gm % and 16 were within the optimal range. All solutions were within the safe and effective range. Field testing confirmed the necessity of supervision and that at least 1 person at the village level should be formally taught to discern the correct size of utensils so that the method may be taught to mothers. Infants receiving these solutions should also receive adequate amounts of free water and potassium rich foods. Until premixed packets are available this method provides an adequate alternative." @default.
