It is estimated that 60% of the population of developing countries require mother and child health care services. To illustrate the statistics the story of a Panamanian family is recounted. The story shows how Western midwives can make a substantial contribution in rural health. Pedro and Maria lived together with their 14-month old son Jose in a small mud/grass hut in the mountain village of Cerro Inglesia Panama some 5 hours from Tole the nearest small town. Pedro was a subsistence farmer who worked long hours in the fields about a 90 minute walk from their home. His crops were rice and corn the staple diet of the area and he also grew root vegetables such as yuca and nami which together with beans made up the family diet. Fruit such as oranges mangos and avocado pears were available when in season. Jose ate some food from his parents plate but relied on breast milk as his main food source. He had grown satisfactorily for the 1st 6 months of his life while breast milk was sufficient to meet his needs but during the past 8 months had gained little weight due to the lack of suitable weaning foods together with repeated respiratory infections gastroenteritis and intestinal parasites. As Maria believed at this time that she was pregnant again she stopped breastfeeding in accordance with the taboo that a lactating mother must not breastfeed while pregnant. Joses condition worsened because of the sudden introduction of new foods prepared in less than hygienic condtions plus the feeding bottle. Pedro took Jose to the health center in Tole. The doctor prescribed a broad spectrum antiparasitic and advised on the necessity of improving the childs weight and completing his immunization program. As the doctor did not have the facilities and equipment for Marias 1st antenatal visit they had to go to the health center in San Felix. At delivery Maria experienced complications. In Tole the doctor diagnosed retained placenta with secondary infection. The story illustrates the need that exists for a more appropriate health care system in most rural areas of the 3rd world. Unskilled midwifery is the only type of care available to 91% of the women giving birth in this area of Panama. The main role of the midwife is education. Once trained the village health promoter or village midwife will be in a position to help the community better understand the principles involved in the prevention of disease and help to dispel harmful superstitions while reinforcing those which are helpful." @default.
