- W2277479339 abstract "The diagnosis of mild cognitive impairment (MCI) heavily relies on clinicians' subjective judgments aided by different tests which require interpretation. To prove the effectiveness of Fisher linear discriminant in differentiating MCI subjects from normal subjects and impartially determine the diagnosis of MCI using only neuropsychological tests without measuring of activities of daily living. Subjects diagnosed with amnestic MCI and control subjects were enrolled over a two-year period. In the first year, there were 41 control subjects and 37 MCI subjects. In the second year, there were 48 control subjects and 26 MCI subjects. Control and MCI subjects were administered the following assessments at annual visits: Geriatric Depression Scale (GDS), Mini-Mental State Examination (MMSE), Rey Auditory Verbal Learning (RAVLT), and CERAD neuropsychological battery (including learning and immediate word recall, object naming, constructional praxis, and word fluency, 60-item Boston Naming Test, Hachinski Ischemic Scale, and Blessed Dementia Rating Scale). Based on 33 neuropsychological tests, the authors applied the Fisher linear discriminant combined with principle component analysis (PCA) to differentiate control subjects from MCI. Data from the first year were applied as training to obtain necessary parameters and data from the second year were applied to test the diagnosis. A Fisher Discriminant Index (FDI) was established to quantify the degree of dementia. An FDI threshold was set to separate MCI subjects from normal controls. The FDI for any given neuropsychological test battery separated MCI from normal with a much higher accuracy than an individual test in that battery. Both the RAVLT and CERAD batteries achieved high accuracy (Table 1). When all the 33 tests were applied, this method provided an even high accuracy with an area under Operating Characteristic (ROC) curve of 0.986. Application of the Fisher linear discriminant can objectively and accurately assess MCI from normal controls based on the overall neuropsychological tests rather than subjective selection of a single test and the interpretation of activity of daily living. Results in terms of sensitivity, specificity, and ROC curve demonstrated that this method is highly accurate." @default.
