- W2277954804 abstract "156 Objectives Rubidium-82 (Rb-82) PET is commonly used to evaluate regional perfusion defects for the diagnosis of coronary artery disease in clinical practice. Recent data have demonstrated accurate and reproducible quantification of myocardial blood flow (MBF) using Rb-82 two-dimensional (2D) PET. Three-dimensional (3D) data acquisition is standard with current PET/CT scanners and is advantageous in reducing Rb-82 doses and operational costs. However, quantification of MBF with 3D data is challenging due to increasing scatter, and has not been extensively validated. The aim of this study was to evaluate the accuracy of 3D Rb-82 PET derived MBF using O-15-water PET as a non-invasive standard. Methods 11 healthy volunteers (mean age: 32.0±11.8 y) underwent rest and pharmacological stress studies with 3D Rb-82 and 2D O-15-water PET within a 2-month interval (mean 15.3±18.2 days). MBF was estimated from the image derived time-activity curves using a one-compartment model for Rb-82 and O-15-water kinetics. Results Rest MBF, stress MBF, and coronary flow reserve (CFR) were similar between Rb-82 and O-15-water (Rest: 0.65±0.16 vs. 0.67±0.13, Stress: 2.95±0.73 vs. 3.28±1.10, CFR: 4.81±1.75 vs. 4.93±1.46, P=NS). MBF estimated with 3D Rb-82 was correlated with O-15-water PET (R=0.85, P Conclusions MBF measured with Rb-82 and 3D PET was correlated over a wide range of flows with 2D O-15-water PET as a standard MBF measurements.
