- W2278832212 abstract "1915 Objectives We recently showed that expansion of brain hypometabolism followed the anatomical pattern of amyloid deposition with temporal delay in early AD. Based on these findings we now examined longitudinal anatomical changes in cortical atrophy and hypometabolism in relation to amyloid-deposition in early AD patients. Methods 15 early AD patients (68 ± 8 yrs), which underwent baseline (BL) and follow-up (FU) T1w structural MRI, [18F]FDG- and [11C]PIB-PET (FU 27 months) and 15 controls (64 ± 5 yrs), which had undergone identical imaging procedures once, were selected. SPM5 voxelwise VBM and PET correlations in patients relative to controls and between patients’ longitudinal data were performed. The same statistical threshold p Results Relative to controls, patients showed AD-typical patterns of atrophy, hypometabolism- and amyloid-deposition, while BL-amyloid-deposition was more extended than hypometabolism and atrophy. Expansion of cortical atrophy occurred in right PCC, bilateral parietal-, lateral temporal-, and prefrontal cortex, showing high overlap with BL-amyloid-deposition but moderate overlap with BL-hypometabolism. While amyloid deposition remained relatively stable, expansion of cortical atrophy and -hypometabolism was more distinct and occurred partially independent from each other. Conclusions As previously demonstrated for hypometabolism, expansion of brain atrophy in AD patients roughly followed the pattern of BL-amyloid-deposition, indicating a possible causal relation of regional amyloid pathology and subsequent neuronal loss. However, longitudinal atrophy and hypometabolism occurred partially independent, i.e. in regions unaffected from the respective other pathology. These findings may have several explanations: a) Age-associated brain atrophy occurring independently from neurodegeneration; b) Presence of compensatory brain effects, i.e. upregulation of neuronal function (metabolism) in some regions affected by neuronal loss" @default.
