In Sweden in 1880 and in Bangladesh in 1979 maternal mortality was about the same at 600/100000 live births. In 1870 in Sweden 90% of children who mothers died also died before 5 years of age while in Bangladesh the figure was 95% in 1970. Lifetime risk for maternal mortality in Bangladesh is 1/26 compared to 1/10000 in Sweden. Under-reporting is rampant: in Indian state of Andhra Pradesh only 35% of maternal deaths were officially registered according to a study. Among 284 cases 140 mothers were transported to the hospital in a moribund state and only 8% arrived by car. Puerperal mortality reached 69%. 27 (9.5%) of 284 deaths were abortion-related. 41% of deaths were clearly preventable 37% were presumably preventable and 22% were not preventable. In some African countries preventable maternal mortality reaches 80-90%. Most maternal mortality in developing countries is caused by eclampsia bleeding and sepsis with abortion-related mortality being dominant in urban environments. The majority of maternal mortality cases were associated with wanted pregnancy. 83% of women in Africa wanted their last child. Most maternal mortality occurs among women with low obstetrical risk. The first referral level recommended by the HO could save lives: access to surgery at the health center level with anesthesia medical treatment blood transfusion manual obstetric intervention family planning a home for mothers and basic neonatal care. The threat of HIV infection lingers as 20-30% of women in some African metropolises have been shown to be infected. The previous risk group of prostitutes has been replaced by pregnant women.
