Of those patients for whom followup information was available 43 per cent have produced a pregnancy. Semen analyses after vasovasostomy revealed good sperm counts for the most part but generally reduced sperm motility and viability. Efforts are underway to evaluate whether the defect may be the result of abnormal epididymal function or of sperm agglutinating or immobilizing antibodies.139 of 205 men undergoing bilateral vasovasostomy over an 8-year period were available for follow-up and were studied to determine the quality of semen after reversal procedures. 60 pregnancies (43%) were reported. These men were asked to return and provide semen for analysis; however, it is noted that men who have failed to conceive were more likely to return for further study, and the data reflect this. The average age of men having had vasovasostomy was 35.9 years; average interval from sterilization to reversal was 7.6 years. The postoperative sperm count averaged 30.5 million/ml of ejaculate in 77 patients after reversal. Of 48 semen samples analyzed from 16 patients in the series, sperm counts were greater than 20 million in 76%. However, sperm motility was reduced, with only 8% showing normal sperm motility. About 1/2 of the samples showed normal sperm morphology, and about 15% of the samples revealed some agglutination associated with decreased sperm motility. Therefore, the basic defect in reversal semen samples is reduced sperm motility and viability; whether these findings can be attributed to epididymal defects or autoantibody formation is not determined." @default.
