• W2284452008 abstract "51세 남자 환자가 약 4일간의 찌르는 듯한 복통과 혈변을 주 소로 본원에 내원하였고 과거력 상에서 약 4개월 정도 모호한 복통이 있었으나 그 외에 특별한 의학적 기왕력은 없었다. 이학 적 검사에서 우측 복부에 압통과 반발 압통이 있었고 복부 경직 이 관찰되었다. 검사 소견에서 빈혈(Hb 10.1 g/dL)과 직장검사 에서 잠혈(759 ng/mL)이 발견되었고 백혈구 수치(21850/μL) 와 C반응 단백질 수치(259.6 mg/L)가 상승되어 있었다. 내원당시 시행한 조영증강 복부 전산화단층촬영(computed tomography; 이하 CT) 문맥기 영상에서 공장 부위의 소장벽이 두꺼워지고 다른 정상 장벽에 비하여 조영증강이 감소되어 저음 영으로 보이는 소견이 있어 허혈성 장염이 의심되었다. 또한 회장 분절에 결절성 벽비후와 비교적 균일하나 정상 소장벽의 조영증 강 정도에 비하여 조영증강이 감소되어 보이는 종괴가 관찰되었 고, 축상 CT 영상에서 종괴의 크기는 3.6 × 2.7 cm였다(Fig. 1A). 이와 동반되어 우측 부신에 불균일한 조영증강이 되는 등음 영의 고형 성분으로 두 개의 종괴가 관찰되었고 크기는 각각 약 3.1 cm와 3.2 cm였다(Fig. 1B). 그 외 복강 내 다른 부위에 전 이는 보이지 않았다. 환자는 장간막 허혈증으로 항응고치료를 시행하였지만 증상 이 악화되어 내원 80일 후 추적관찰 CT를 시행하였다. 조영증 강 복부 CT에서 회장 종양은 6.5 × 4.2 cm로 크기가 증가되고 Undifferentiated pleomorphic sarcoma (UPS), previously known as malignant fibrous histiocytoma, is a soft tissue sarcoma arising from mesenchymal tissue of the body. UPS of the gastrointestinal tract is known to be rare and only a few cases have been reported in the literature. Based on our case and review of the other relevant literature, the CT findings of primary UPS of the small bowel included nodular bowel wall thickening with homogeneous enhancement. It presents as a rapidly growing tumor without bowel obstruction, and it may be accompanied by distant metastasis." @default.
