- W2285958654 abstract "Estroprogestin-type oral contraceptives (OCs) induce neuro- ophthalmological side effects of varying severity that can be reversible or irreversible. A 20-year old woman with temporary loss of vision since July was examined in September 1987. Anamnesis revealed a family history of headache in the father and paternal grandmother as well as OC (combination of .030mg ethinyl estradiol and .15mg desogestrel) use of 3 months duration. Ophthalmological examination indicated the existence of a left homonymous hemianopsia in the visual field. Neurological examination was normal except for the field-related deficit. Cardiological examination was norma but electrocardiogram (ECG) showed a focal block of the right branch. Nuclear magnetic resonance (NMR) of the head showed altered signals in the right region interpreted to be of vascular origin. Carotidography and vertebralgraphy indicated an avascularized zone with obstruction of a small branch of the artery at the level of the occipital lobe at an inter-hemispheric site. Piruvic acid and lactic acid were normal excluding a pathology of metabolic origin. The presence of ischemic inflammation in the territory of the right posterior cerebral artery was diagnosed. 1 cpr/day idobufene treatment was initiated and OC use was halted. Periodic controls showed progressive return of normal parameters: beta- thromboglobulinemia from 106 ng/ml to 67 ng/ml; fibrinopeptide A to 13 ng/ml; platelet factor to 18.3 ng/ml; and platelet adhesion to 15%. OC use has been associated with thromboembolic disorders in young women leading to a 2-10 times higher incidence of ictus cerebri than in nonusers although OCs with low-dose estrogen reduce this risk." @default.
