- W2287515266 abstract "Background: Primary hepatobiliary tumors are rare in dogs and cats. Studies suggest a prevalence of 0.6% in dogs and 2.9% in cats from all occurring tumors. Neoplasia in these tissues can have hepatocellular and bile ducts origin or even be sarcomas or neuroendocrine tumors. Its clinical signs are nonspecific and the diagnosis is most often tardy or even done during necropsy. A case of cholangiocellular carcinoma and glomerulonephritis in a cat is reported, addressing its clinical aspects and emphasizing the importance of an early diagnosis. Case: A mixed breed, 12-year-old neutered cat was admitted in the Veterinary Teaching Hospital of the Federal University of Rio Grande do Sul treated previously without obvious improvement. Jaundice, severe dehydration, excessive salivation, low body condition (score three out of nine), abdominal discomfort, and lethargy were evident on physical examination. Blood tests showed neutrophilic leukocytosis, lymphopenia, and increased serum levels of both alanine aminotransferase and alkaline phosphatase enzymes. Abdominal ultrasound examination revealed a large single mass of three centimeters of diameter, approximately, compressing the bile ducts. Because of the severity of the patient’s condition, it was hospitalized to receive supportive care. During hospitalization, the animal received feeding through an esophageal tube, intravenous fluid therapy, antibiotics, antiacids, antiemetic medication, analgesics, vitamins and amino acids supplementation. Moreover, due to excessive salivation, which is one of the signs of hepatic encephalopathy, lactulone and metronidazole was administered as preventive treatment. After stabilization, a laparotomy was performed. As the gallbladder was incised a mass was observed obstructing the bile flow. Severe changes were also observed in the hepatic parenchyma. Facing the nature of the tumor and the debilitating situation of the patient, euthanasia was performed immediately after the surgical procedure. Necropsy and histopathological analysis established the final diagnosis of cholangiocellular carcinoma and glomerulonephritis. Discussion: Cholangiocellular carcinomas are malignancies originated from the biliary tract epithelium and usually show very nonspecific clinical, hematological, and biochemical signs. Therefore, in most cases, these tumors are much developed at diagnosis. Corroborating the data from literature, the patient had pain on abdominal palpation, leukocytosis and elevated serum levels of alkaline phosphatase. The late diagnosis obtained by ultrasonography, along with the unfavorable progression of the pacient’s condition, hindered the treatment and made euthanasia the elected procedure. Although some causes of experimental and naturally occurring hepatobiliary tumors have been identified in other species, they are not well defined in cats. Even that biliary tract parasites, such as Platynosomum fastosum, and chronic inflammation of the hepatobiliary parenchyma have been associated with hepatobiliary tumors in this specie, these were not found at necropsy. Renal histopathological findings were consistent with glomerulonephritis; however, the animal showed unspecific clinical signs and further investigations for renal disorders were not performed. According to the literature, cholangiocarcinomas are not considered a cause of glomerulonephritis in cats, so, although the liver and kidney disorders occurred concomitantly, it is possible that they were unrelated diseases." @default.
