La prévalence du syndrome de stress post-traumatique (Post-Traumatic Stress Disorder [PTSD]) chez les patients atteints de douleur chronique montre qu’il existe une relation bi-directionnelle et un entretien mutuel (mutual maintenance) entre ces deux entités. Si dans une proportion significative, des événements traumatiques et des chocs émotionnels sont repérables dans les antécédents du patient douloureux, nous souhaitons ici interroger d’un point de vue psychodynamique les liens entre la douleur chronique et un type d’événement singulier : l’épreuve du deuil. Nous proposons de nous interroger sur la vulnérabilité somatique induite par la perte et la façon dont la douleur va occuper l’espace corporel fragilisé pour s’y installer. De quelle façon le processus intrapsychique consécutif à l’expérience de la perte peut-il ne pas conduire à une élaboration du deuil mais se modifier en termes d’installation du syndrome douloureux chronique ? À partir de trois vignettes cliniques, les relations entre la chronicisation des douleurs, l’omission du travail de deuil et la pertinence clinique de la notion de crypte douloureuse seront examinées. La douleur chronique sera alors située comme l’indice de la persistance d’un lien à l’objet perdu qui n’a pas pu être élaboré dans un processus séparateur. The prevalence of Post Traumatic Stress Disorder (PTSD) amongst patients suffering from chronic pain shows the existence of a bi-directional relationship and a mutual maintenance between these two entities. If, in a significant proportion, traumatic events and emotional shocks recur in the antecedents of the patient in pain, we hereby wish, to question the links between chronic pain and the particular life event that is mourning from a psychodynamic point of view. We offer to question the somatic vulnerability induced by loss and the way in which pain invades the weakened body and settles there. How could the intrapsychic process following on from experiencing loss not lead to the development of mourning but also change when it comes to chronic pain syndrome surfacing? From three clinical situations, the relationships between pain chronicisation, the omission of the mourning process and the clinical relevance of the painful crypt concept will be examined. Chronic pain will then be used as the index for the persistence of the link to the lost object, which a separating process was not able to elaborate." @default.
