Perhaps a study of HIEs in the jail environment might make the benefits of HIE clearer. Over 11.7 million people cycle through our nation’s jails each year. 1 They have chronic conditions and higher rates of tuberculosis, HIV, hepatitis B and C, arthritis, diabetes, and sexually transmitted diseases, compared to the general population. Sixty-four percent have a mental health disorder. 1With the expansion of Medicaid, many jail-involved individuals are being insured and are seeking treatment from community providers who are using electronic health records that upload data to HIEs. A few jail health providers are using HIE portals at intake to view people’s past medical histories. In Lexington, Kentucky, the electronic health record in the jail is connected to the Kentucky Health Information Exchange and as of April 2, 2015, is fully exchanging data with that HIE (Rodney Ballard, director, Lexington-Fayette Urban County Government Division of Community Corrections, personal communication, April 2, 2015).This example of HIE would be an opportunity to study how HIE can affect the health care of a clearly defined set of people and see whether it increases efficiency, improves quality, and lowers cost.
