- W2289774635 abstract "This chapter discusses the estimation of renal function and identification of patients with renal insufficiency, nutritional assessment of patients with renal failure, and therapeutic principles for dietary management of such patients. Chronic renal failure imposes a wide variety of abnormalities on the body's metabolic machinery. In the middle aged and elderly, the causes of acute renal failure are prerenal states complicating atherosclerotic cardiovascular and peripheral vascular disease, obstructive uropathy from prostatic enlargement or pelvic malignancy, nephrotoxins such as aminoglycoside antibiotics and iodinated radiographic contrast media, and perioperative complications. Nutritional therapy plays a major role in the management of patients with acute and chronic renal insufficiency. In patients with acute renal failure, the principal goals of conservative therapy include minimizing or preventing protein catabolism by prescribing adequate calories and protein and reducing the need for dialytic therapy by tailoring the intake of sodium, potassium, water, alkali, and nitrogen to the patient's needs and excretory capacity. The fundamental principles involved in dietary therapy in acute and chronic renal failure are identical. Patients with renal insufficiency must take in sufficient calories to prevent endogenous protein catabolism that leads to loss of lean body mass and worsening azotemia. Lipid abnormalities are very common in many forms of renal disease, and this contributes to the high incidence of atherosclerotic cardiovascular disease that often complicates renal failure." @default.
