• W2291978259 abstract "Приведен клинический пример лечения пациентки с парапротезной инфекцией в области коленного сустава. Выполнено оперативное вмешательство: удаление эндопротеза левого коленного сустава, некрэктомия костей коленного сустава по типу резекции 7 см, дренирование. Остеосинтез бедра и голени аппаратом Илизарова Особенностью оперативного вмешательства являлась резекция в пределах здоровой кости, с подработкой бедренной и большеберцовой костей по типу «лунка – сфера» (в мыщелках бедренной кости сформирована лунка, проксимальный конец большеберцовой кости подработан до конгруэнтности под полусферу). В послеоперационном периоде проведен повторный курс антибиотикотерапии. Заживление раны первичным натяжением, свищи закрылись. Через 1,5 месяца после купирования гнойного процесса в области коленного сустава в отсроченном порядке выполнена операция: остеотомия нижней трети левой большеберцовой кости с целью замещения дефекта. Осуществлялась методика билокального компрессионно-дистракционного остеосинтеза на левом бедре и голени в течение 127 дней. Фиксация в аппарате Илизарова – 174 дня. Достигнут положительный результат. The work deals with a clinical example of treating a female patient with periprosthetic infection in the knee area. The following surgical intervention performed: removal of the left knee implant, necrectomy of the knee bones by resection type (7 cm), draining. Osteosynthesis of the femur and the leg using the Ilizarov device carried out. The characteristic feature of the performed surgery consisted in making resection within normal bone, as well as forming the femur and leg bones of “fossa-sphere” type (a hollow formed in the femoral condyles, and the tibial end fitted to the congruence with the hemisphere). A second course of antibiotic therapy used postoperatively. Wound healing occurred by first intention, fistulas closed. The following delayed surgery performed 1.5 months after the purulent process was stopped in the knee: osteotomy of the left tibia in its lower third in order to fill the defect. The technique of bilocal compression-distraction osteosynthesis in the left femur and leg implemented for 127 days. Fixation with the Ilizarov device lasted for 174 days.
